  • Description: Provides a two-part comprehensive health consultation for older adults with complex, chronic conditions.The goal of the program is to help older adults live more productive, healthy lives and to help families with the difficult tasks of providing for care. For the first part of the consulation, patients meet with a geriatrician, neuropsychologist, social worker, occupational therapist and dietitian. During this meeting, the team reviews medical records, prescriptions, living situation and more in order to identify any issues the patient may be having. About two to three weeks after the initial meeting, the second part of the consultation invovles meeting with the patient and their family to review recommendations made by the team. The recommendations can be shared with the patient's healthcare provider.
